Job summary

AdvisaCare is seeking a per diem MSW or BSW to join our team in Mount Pleasant, Michigan. The successful candidate will be responsible for conducting assessments, developing care plans, and providing counseling to patients and families dealing with life-limiting illnesses. Applicants should have a graduate degree in social work, with a preference for candidates who have home health care and hospice experience. This position offers a supportive work environment with flexible scheduling and excellent pay.

Qualifications

• MSW degree from a school of social work accredited by CSWE.
• One year of experience in Home Health Care preferred.
• Hospice experience preferred.

Responsibilities

• Perform psychosocial, emotional, spiritual, and bereavement assessments.
• Document patient visits and referrals accurately.
• Provide crisis intervention and counseling as needed.

AdvisaCare Home Health and Hospice is looking for a per diem MSW (Masters of Social Work) or BSW (Bachelors of Social Work) to join our team in the Alma and Mt Pleasant areas with our Troy location being your Parent Agency. Requirements

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• One year of experience in Home Health Care preferred
• Hospice experience preferred
• Valid Driver's License, car insurance and reliable transportation

Job Duties

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans
• Provides care under the direction of a physician. Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family
• Documents all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record
• Conducts reassessments of patient/family needs and counseling as required
• Provides crisis intervention and individual or family counseling when indicated
• Acts as a care advocate for the patient/family unit
• Develops and maintains contact with appropriate community agencies and services to promote interagency cooperation and facilitate referrals
• Provides ongoing counseling related to issues of death and dying to the patient and family as needed
• Participates as a member of the Bereavement Team as assigned
• Adheres to all Hospice policies
