Qualifications

• NH Physical Therapist License required.
• Basic Life Support (BLS) certification required within 2 weeks of hire.

Responsibilities

• Establish therapy goals and formulate a treatment plan based on evaluation results.
• Supervises PTAs according to organization policy and state regulations.
• Provides individual, family and group therapy sessions for clients engaged in outpatient therapy.
• Participates in interdisciplinary group for hospice patients and case conference meetings for home health patients.
• Participates in discharge planning.
• Records all pertinent data in the patient's medical records.
