Qualifications

• Experience providing care to diverse patient populations across age groups including neonate to geriatric.

Responsibilities

• Reviews orders to determine therapeutic appropriateness and contacts prescribers as needed.
• Enters medication orders in the pharmacy information system with correct formulation and dosing.
• Dispenses medications accurately and monitors drug therapy for safety and efficacy.
• Educates healthcare staff and patients about drugs, interactions, and administration.
• Participates in quality improvement and staff development activities.
