Job summary

Parkside Health and Rehabilitation Center is seeking a compassionate Certified Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team on a per diem basis. The OT will evaluate residents, develop individualized treatment plans, and provide therapy to promote independence and safety.

Responsibilities include ADLs training, functional mobility, gait, coordination, and cognition while ensuring Medicare and facility guidelines are followed.

Qualifications

• Master’s or Doctorate degree in Occupational Therapy from an accredited program.
• Current OT license in good standing in Florida.
• Long‑term care or skilled nursing experience preferred.
• Knowledge of geriatric rehabilitation and Medicare regulations.
• Strong communication and documentation skills.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide therapy focused on ADLs, mobility, strength, coordination, cognition and safety.
• Monitor progress and adjust treatment plans as needed.
• Complete accurate documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with regulations.
