Job summary

Sandgate Gardens Rehab and Nursing Center in Florida is seeking a compassionate Certified Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team on a per diem basis. The OT will evaluate residents, create individualized treatment plans, and deliver therapy to promote independence, safety, and improved quality of life.

The role involves ADL training, functional mobility work, and collaboration with an interdisciplinary team.

Qualifications

• Master’s or Doctorate in Occupational Therapy from an accredited program.
• Current Florida OT license in good standing.
• Experience in skilled nursing or long-term care preferred.
• Knowledge of geriatric rehabilitation and Medicare regulations.
• Strong communication and documentation skills.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide therapy focused on ADLs, functional mobility, strength, coordination, cognition, and safety.
• Monitor resident progress and adjust treatment plans as needed.
• Complete accurate and timely documentation in compliance with Medicare and facility guidelines.
• Participate in interdisciplinary care planning and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.
