Job summary

Willow Creek Health Care Center in Clovis, CA is seeking a Per Diem Occupational Therapist to join our rehabilitation team. You will evaluate and treat residents, communicate with families and physicians, and maintain accurate documentation of services.

The role includes supervising COTAs and therapy aides, contributing to care plans, providing staff education, and promoting safety and independence through ADL-focused therapy while complying with regulatory standards.

Qualifications

• Bachelor's Degree in Occupational Therapy required (Master's or Doctorate preferred).
• Experience in a skilled nursing facility preferred.
• Proficiency with Net Health and PointClickCare documentation systems.

Responsibilities

• Evaluate residents to determine OT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve ADLs.
• Supervise COTAs, aides, and students within state guidelines.
• Collaborate with physicians and interdisciplinary team; participate in care plan meetings.
• Provide in-services and training for facility staff.
• Maintain timely, accurate documentation in the medical record.
• Ensure safe treatment environment and infection control.
• Educate residents and families on therapy goals and home programs.
• Recommend adaptive equipment as needed.
• Participate in quality improvement initiatives.
