Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a Per Diem Occupational Therapist to join our rehab team. The OT will evaluate residents, develop treatment plans, and deliver interventions to improve independence in daily living and mobility.

This role offers flexible per diem scheduling, competitive pay of $60–$65 per hour BOE, and opportunities for ongoing training, collaboration with physicians and staff, and commitment to patient-centered care.

Qualifications

• Bachelor's Degree in Occupational Therapy required.
• Master's or Doctorate preferred.
• Current California OT license in good standing.

Responsibilities

• Evaluate residents and determine OT needs and rehab potential.
• Develop, implement, and modify individualized treatment plans.
• Provide therapeutic interventions focused on ADLs, strength, balance, cognition.
• Communicate with physicians, nursing staff, residents, and families regarding progress and goals.
• Participate in care plan meetings and interdisciplinary team conferences.
• Complete timely, accurate, and compliant documentation.
• Educate residents and caregivers on home programs, adaptive strategies, and fall prevention.
• Recommend adaptive equipment and assistive devices to promote independence.
• Supervise COTAs and therapy aides as appropriate.
• Maintain a safe treatment environment and follow infection control procedures.
