Job summary

Willow Creek Health Care Center is seeking a compassionate per diem Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in California. Under the supervision of a licensed OT, you will deliver skilled treatment to help residents regain independence in daily activities and improve functional quality of life.

Responsibilities include implementing the plan of care, assisting with mobility and self-care activities, documenting progress, educating residents and families,

Qualifications

• OTA degree from an accredited program.
• Current California OTA license in good standing.
• CPR certification when required by policy.
• Experience in skilled nursing or post-acute rehab preferred.

Responsibilities

• Provide OT treatment per plan of care under supervision.
• Assist residents with mobility, ADLs, and safety tasks.
• Document progress timely and accurately.
• Collaborate with physicians, nurses and rehab staff; participate in care conferences.
