Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a Per Diem Occupational Therapist to join our compassionate rehab team. The OT will assess residents, develop individualized plans, and provide therapeutic interventions to enhance daily living, mobility, safety, and independence.

The position offers competitive pay of $60–$65/hour, a flexible per diem schedule, ongoing training, and a supportive, resident-centered culture aimed at helping residents regain independence.

Qualifications

• Bachelor’s Degree in Occupational Therapy is required (OT).
• Master’s or Doctorate preferred.
• California OT license in good standing.
• Experience with Casamba and PointClickCare is preferred.
• Strong communication and interpersonal skills.
• Patient-centered care and functional outcomes.

Responsibilities

• Evaluate residents and determine OT needs and rehab potential.
• Develop, implement, and modify individualized treatment plans.
• Provide interventions focused on ADLs, strength, balance, cognition, and safety.
• Communicate progress with physicians, nursing staff, residents, and families.
• Participate in care plan meetings and interdisciplinary conferences.
• Complete timely, accurate documentation.
• Educate residents and caregivers on home programs, adaptive strategies, and fall prevention.
• Recommend adaptive equipment and devices to promote independence.
• Supervise COTAs and therapy aides as appropriate.
• Maintain a safe treatment environment and infection control procedures.
