Job summary

Hartford HealthCare is seeking a Per Diem Occupational Therapist for our Outpatient clinic in Manchester, CT. The OT independently evaluates patients’ function and develops individualized treatment plans aligned with patient goals to promote independence in daily living and productive activities.

Qualifications

• Graduate of Occupational Therapy program with a BS or entry-level master's.
• CT active or temporary OT license.
• CPR certification is required.

Responsibilities

• Performs thorough evaluation of patient function after physician referral.
• Establishes measurable goals with patient/family based on care protocols.
• Delivers interventions to maximize daily living and functional independence.
• Provides patient and caregiver education on treatment and equipment use.
• Completes documentation including evaluation, care plan, progress and discharge.
• Coordinates care with Rehab Therapists, Nurses, Social Workers and others.
