Job summary

Hartford HealthCare is seeking a per diem Occupational Therapist to independently evaluate patients and develop individualized goals and treatment plans aligned with patient and family objectives.

The role includes delivering OT services to improve daily living, recreation, and productive work skills, delegating tasks to supervisee staff, and coordinating with the broader care team to ensure smooth transitions across care settings.

Qualifications

• ACOTE-accredited OT program degree required (Bachelor/Master/Doctorate).
• CT license to practice OT and NBCOT certification required.
• New graduates welcome; experience preferred.

Responsibilities

• Perform thorough evaluation of patient level of function and establish measurable goals with patient/family.
• Develop and implement treatment plans aligned to patient and caregiver goals.
• Deliver OT services to maximize independence in daily living, leisure, and work activities.
• Provide patient and caregiver education on treatment, precautions and equipment.
• Complete documentation including evaluation, care plan, progress notes, discharge summary and transition plan.
• Coordinate care with Rehab Therapists, Nurses, Social Workers and other care providers during transitions.
