Flagship Rehabilitation, part of Enhance Therapies, is seeking an Occupational Therapist for a per diem role in Lake Ridge, VA. You will evaluate and treat patients in SNF/long-term care settings, creating personalized care plans and collaborating with a multidisciplinary team to ensure high-quality outcomes.

Applicants must have an OT degree from an AOTA-accredited program, hold NBCOT certification if required by state, and be a licensed OT in the state.

Qualifications

• Degree in Occupational Therapy from an AOTA-accredited program.
• NBCOT certification as an OT may be required by the state.
• Current license as an Occupational Therapist in the state of practice.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
