Job summary

LiveWell Homecare Agency, LLC is seeking an Occupational Therapist Assistant for a per diem home health role. The clinician will deliver patient-centered therapy in patients’ homes under supervision, following established treatment plans and state regulations.

You will educate patients and caregivers to promote independence and recovery. Responsibilities include assessing progress, documenting responses, and communicating updates to the supervising therapist and physician.

Qualifications

• Current Occupational Therapy Assistant licensure in the state of practice.
• NBCOT certification (state-specific) is required or in place.
• CPR certification or ability to obtain within 90 days (preferred).
• Valid driver’s license, reliable transportation, or access to public transit.

Responsibilities

• Providing occupational therapy services in patients’ homes under supervision, adhering to state laws and treatment plans.
• Instructing and aiding patients, families, and healthcare team members in evidence-based therapy within scope.
• Observing, documenting, and reporting patient responses and condition changes to supervisors and physicians.
• Demonstrating cultural competence and ability to work with diverse populations.
