Job summary

JAG Physical Therapy in New Jersey seeks a Per Diem Physical Therapist to join our care-first outpatient team. You will assess patients, develop and implement treatment plans, and document progress using Prompt EMR, while maintaining patient confidentiality and a positive, professional demeanor.

This role supports coverage for vacations and leaves, collaborating with a multidisciplinary team to maximize functional independence and patient outcomes.

Qualifications

• Bachelor's or Master's degree required; Doctorate degree preferred.
• Licensed Physical Therapist in NJ.
• Completed CEU requirement in NY/NJ.
• Valid CPR license.
• Experience with EMR software preferred.
• Excellent oral and written communication.

Responsibilities

• Initial evaluation and daily notes.
• Reassessments, progress reports, and discharge plans.
• Documentation with billing following department guidelines.
• Obtain patient history and research diagnoses as needed.
• Establish treatment plan and goals with interdisciplinary team.
• Provide education to patient and family; perform functional training.
• Deliver musculoskeletal interventions and exercise prescriptions.
• Utilize Prompt EMR for notes and attend billing seminars.
