JAG Physical Therapy is seeking a Per Diem Physical Therapist to join our outpatient team. The PT will conduct evaluations, develop treatment plans, and implement care for patients, working with an interdisciplinary team to achieve functional goals.

Licensed PT in NJ is required with CEU compliance; EMR experience is preferred. The role supports staffing coverage for vacations and leaves, emphasizing patient-centered care in a fast-paced environment.

Qualificações

• Requires a PT license in the state
• Bachelor's or Master's degree required; Doctorate preferred
• CEU requirements met in NY/NJ as applicable

Responsabilidades

• Collect and record patient care data per guidelines and reimbursement impact
• Perform initial evaluation, daily notes, progress reports, and discharge plans
• Record daily billing and support productivity guidelines
• Prepare for evaluations per department policies and standards
• Select and administer tests and measures; synthesize results for diagnosis and plan of care
• Establish treatment plan based on evaluation and goals; collaborate with team
• Identify need for consultations and discharge planning
• Provide patient education and functional training for ADLs/IADLs
