Responsibilities

• Provide ongoing assessment of nursing care needs for a defined group of patients.
• Develop a written plan of care based on nursing diagnosis and patient needs.
• Assist patients and families in developing health care goals as part of the care plan.
• Provide nursing interventions safely and competently.
• Offer telephone consultation to patients and families.
• Evaluate patient responses to care and modify care plans as needed.
• Communicate effectively with patients and families regarding their psychosocial and emotional needs.
• Develop educational components for nursing care plans and provide teaching/counseling.
• Promote patient self-care responsibility and adherence to health standards.
• Participate in continued education programs and nursing research.
