Job summary

Responsibilities

• Provide ongoing assessment of nursing care needs for a defined group of patients.
• Develop a written plan of care based on nursing diagnosis and patient needs.
• Assist patients and families in developing health care goals as part of the individualized plan of care.
• Provide nursing interventions safely and competently, anticipating changes in patient status.
• Demonstrate understanding of the physiological and psychological effects of normal aging.
• Collaborate and communicate with healthcare team members in planning and implementing care.
• Assess patient lifestyle risk factors and integrate health promotion into care.
• Provide teaching and counseling congruent with patients' learning needs and medical plans.
• Participate in continued education programs and staff orientation activities.
• Assure quality of care through the development and evaluation of nursing standards.
