Job summary

Avir Trilogy is seeking a dedicated Per Diem / PRN Occupational Therapist (OT) to join the rehabilitation team at Avir at Montana in El Paso, TX. The OT will evaluate and treat patients with diverse functional and rehabilitation needs to enhance independence and daily functioning.

Responsibilities include developing treatment plans, delivering skilled OT interventions, documenting progress, and collaborating with an interdisciplinary team.

Qualifications

• Current Occupational Therapist license in Texas.
• Graduate of an accredited Occupational Therapy program.
• Current CPR certification, if required by facility policy.
• Experience in skilled nursing, rehabilitation, long-term care, or related healthcare setting preferred.
• Strong clinical, communication, organizational, and documentation skills.
• Ability to work independently and effectively as part of an interdisciplinary team.
• Dependable, flexible, compassionate, and committed to providing high-quality patient-centered care.

Responsibilities

• Evaluate patients' functional abilities and develop individualized OT treatment plans.
• Provide skilled OT interventions based on diagnoses, conditions, and goals.
• Address activities of daily living, strength, coordination, balance, mobility, cognition, and functional performance.
• Monitor patient progress and adjust treatment plans as appropriate.
• Complete accurate evaluations, treatment notes, progress reports, and discharge documentation.
• Collaborate with physicians, nursing staff, PTs, SLPs, and other team members.
• Educate patients, families, and caregivers on exercises, techniques, safety, and home programs.
• Participate in care planning and interdisciplinary meetings as needed.
• Maintain compliance with federal/state regulations, facility policies, and professional standards.
• Provide coverage on a PRN/per-diem basis according to facility and patient needs.
