Responsibilities

• Evaluate patients' physical and functional abilities and develop individualized treatment plans.
• Provide skilled physical therapy interventions based on each patient's diagnosis, condition, and rehabilitation goals.
• Treat mobility, strength, balance, coordination, endurance, gait, and functional limitations.
• Provide therapeutic exercises, functional mobility training, gait training, and other appropriate skilled interventions.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Complete accurate and timely evaluations, treatment notes, progress reports, and discharge documentation.
• Collaborate with physicians, nursing staff, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team.
• Educate patients, families, and caregivers regarding exercises, mobility techniques, safety precautions, and home programs.
• Participate in care planning and interdisciplinary meetings as needed.
• Maintain compliance with applicable federal and state regulations, facility policies, and professional standards.
• Provide coverage on a flexible PRN/per-diem basis according to facility and patient needs.

Position Summary

Avir at Montana is seeking a dedicated and compassionate Per Diem / PRN Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist will provide skilled evaluation and treatment services to patients with a variety of mobility, strength, balance, and functional limitations.
