Southwoods Health at Home is seeking a Per Diem Occupational Therapist to perform patient-centered evaluations and treatments in home health in Boardman, Ohio. You will administer therapy, assess needs, and develop care plans in collaboration with a multidisciplinary team.

Responsibilities include documenting care, educating patients and families, and coordinating with physicians. AHA BLS cert and OT degree are required; prior OT practice is preferred.

Formación

• Graduate of an accredited school for Occupational Therapy.
• Current AHA BLS certification.
• Experience as a practicing Occupational Therapist (preferred).
• Strong knowledge of managing patients with upper extremity weakness, ADL deficits.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary team.

Responsabilidades

• Administers prescribed occupational therapy to restore function and prevent disability.
• Reviews referral orders, patient condition, and records to determine needs.
• Develops and revises treatment plans based on evaluation and status.
• Confers with physicians to obtain information and integrate therapy with care.
• Determines referral needs and makes timely referrals.
• Provides supervision for staff OT assistants and others per regulations.
• Documents patient care in accordance with regulatory requirements and agency standards.
• Educates patients and families on recovery, protocols, treatment, and home programs.
• May plan and conduct in-services or trainings on occupational therapy topics.
• Shows flexibility in work schedule to meet patient needs.
