Job summary

Southwoods Health at Home is seeking a Per Diem Occupational Therapist to perform patient-centered evaluations and treatments in home health in Boardman, Ohio. You will administer therapy, assess needs, and develop care plans in collaboration with a multidisciplinary team.

Responsibilities include documenting care, educating patients and families, and coordinating with physicians. AHA BLS cert and OT degree are required; prior OT practice is preferred.

Qualifications

• Graduate of an accredited school for Occupational Therapy.
• Current AHA BLS certification.
• Experience as a practicing Occupational Therapist (preferred).
• Strong knowledge of managing patients with upper extremity weakness, ADL deficits.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary team.

Responsibilities

• Administers prescribed occupational therapy to restore function and prevent disability.
• Reviews referral orders, patient condition, and records to determine needs.
• Develops and revises treatment plans based on evaluation and status.
• Confers with physicians to obtain information and integrate therapy with care.
• Determines referral needs and makes timely referrals.
• Provides supervision for staff OT assistants and others per regulations.
• Documents patient care in accordance with regulatory requirements and agency standards.
• Educates patients and families on recovery, protocols, treatment, and home programs.
• May plan and conduct in-services or trainings on occupational therapy topics.
• Shows flexibility in work schedule to meet patient needs.

A division of Southwoods Health, Southwoods Health at Home is hiring a Per Diem Occupational Therapist. Will perform patient-centered occupational therapy evaluations and treatments that meet company, federal, state, and local standards, guidelines and regulations.

Essential Duties:

• Administers medically prescribed occupational therapy treatment to patients suffering from injuries or disease to restore function, relieve pain and prevent disability.
• Reviews physician referral/orders, patient’s condition and medical records to determine occupational therapy treatment needs.
• Develops and revises occupational therapy treatment plans as appropriate based on evaluation and continued monitoring of patient status and response to treatment
• Confers with physician and other practitioners to obtain additional patient information, suggest revisions in treatment program and integrate occupational therapy treatment with other aspects of patient’s health care.
• Determines referral needs and makes applicable referrals in a timely manner.
• Provides appropriate level of supervision for staff occupational therapist assistants, students, other licensed personnel and unlicensed personnel according to state and federal requirements.
• Documents patient care services in accordance with regulatory requirements and within time expectation set by agency standards.
• Provides education to patients and patient families as appropriate, including, but not limited to, information on post-op recovery processes, physician protocols, treatment plan, and home exercise programs.
• May plan and conduct in-services or training programs on occupational therapy and related topics for medical staff, students and/or community groups.
• Demonstrates flexibility in work schedule to meet patient scheduling needs.
