Qualifications

• Must be a graduate of an Occupational Therapy program approved by the AMA in collaboration with the American Occupational Therapy Association.
• Preference for American Occupational Therapy Association membership.
• Massachusetts license/registration required; CPR certification current.

Responsibilities

• Provide skilled Occupational Therapy services in patients' homes on an intermittent basis per physician orders.
• Assist physician in evaluating function using diagnostic/prognostic procedures.
• Complete initial and ongoing OASIS assessments and care planning.
• Assess home environment, family support, and community services needs.
• Ensure orders are appropriate and adjust as needed with physician supervisor.
• Develop and implement interdisciplinary OT plans of care with the physician; guide patient in self-care activities.
• Instruct patient in use of wheelchairs, braces, splints, and prosthetic devices.
• Educate and supervise family and patient in OT program and home-related issues.
• Set goals based on current function and potential for improvement; perform re-evaluations as needed.
• Coordinate outpatient OT services when procedures cannot be done at home.
• Document progress notes on visit day or within 72 hours for charting.
