Job summary

Jethro Health provides patient-focused home health care and aims to deliver high-quality outcomes through a multidisciplinary approach. The organization is seeking a per diem Occupational Therapist to manage the OT services for home health patients.

The OT will evaluate, plan, and implement therapy, educate patients and families, and coordinate with the care team to ensure quality outcomes and appropriate discharge planning.

Qualifications

• Must graduate from an Occupational Therapy program approved by relevant accrediting bodies.
• Preferably a member of the American Occupational Therapy Association.
• Licensed or registered by the State of Massachusetts.
• At least one year of institutional experience.
• Current CPR certification.
• Home Health Care: 1 year (Required).

Responsibilities

• Provide skilled Occupational Therapy services in patients' homes per physician orders.
• Assist in evaluating functional status and planning care with the physician.
• Conduct initial and ongoing OASIS-like assessments and update care plan as needed.
• Instruct patients and families in self-care, activities, and use of assistive devices.
• Coordinate with the health care team to develop and implement the OT portion of the plan.
• Prepare progress notes within the visit documentation window.
• Communicate with physicians and team members regularly to optimize patient care.
• Supervise or collaborate with home health aides and occupational therapy assistant per policy.
