As the Per Diem Nurse on our team, you play a vital role in shaping each patient's care; including assessing needs, planning treatment, and evaluating outcomes with compassion and confidence.
During your shift, you will take ownership of delivering high-quality care while staying aligned with all regulatory and documentation standards. You'll collaborate with a multi-disciplinary team, contributing to quality improvement and patient-centered outcomes. The STEP (Skills, Transitions, Exploration and Progress) Program is a partnership between Kaiser Permanente and Seneca Family of Agencies located in San Jose, CA. STEP provides intensive, short-term stabilization services to youth, ages 12-17, for an average of 15 days. Our goal is to reintegrate our youth into schools and the community with lower levels of outpatient mental health treatment.
