Per Diem - Nurse Practitioner or Physician Assistant, Manhattan

This position offers a $10k Sign-on Bonus for External Candidates. Optum Home & Community Care, part of UnitedHealth Group, is innovating in healthcare by creating an integrated care model that addresses physical, mental, and social needs, providing care at home, nursing homes, and assisted living facilities.

As part of our Senior Community Care (SCC) team, you will deliver care to patients at home, supporting access and navigation of healthcare services. We are rapidly expanding and becoming the nation's largest employer of Nurse Practitioners, offering a professional environment with growth opportunities.

Primary Responsibilities include:

  1. Primary Care Delivery
  • Provide cost-effective, high-quality care to assigned members.
  • Manage medical and behavioral, chronic and acute conditions collaboratively with physicians or specialists.
  • Conduct comprehensive assessments, document findings per CMS regulations, and ensure accurate ICD10 coding.
  • Perform medication reviews, develop and revise care plans, and facilitate member engagement.
  • Care Coordination
    • Understand and communicate benefits, policies, and procedures.
    • Assess and coordinate care transitions across settings.
    • Monitor and adjust care plans based on changing needs.
  • Program Enhancement and Communication
    • Maintain effective communication with all stakeholders.
    • Promote the CCM program and participate in quality initiatives.
    • Enter available hours into scheduling systems timely.

    Minimum Qualifications:

    • Certified Nurse Practitioner or Physician Assistant with a national board certification.
    • Active, unrestricted license in your state.
    • Current DEA license or ability to obtain post-hire.
    • Reliable transportation and ability to travel between care sites.
    • Availability to work 24 hours/month, including off-hours, not exceeding 960 hours/year.

    Preferred Qualifications:

    • Over 1 year of post-grad experience in Long Term Care.
    • Knowledge of geriatrics, chronic illness, and end-of-life discussions.
    • Proficient in electronic medical records and developing positive patient relationships.

    The hourly rate ranges from $43.17 to $84.95, based on experience and location. Benefits include comprehensive packages, incentives, stock options, and 401k.
