Job description

Overview

Works as a member of an interdisciplinary care team at a NORC (Naturally Occurring Retirement Community) or Senior Living Retirement Community location. Responsible for coordinating a broad range of health and social services to support older residents in aging in their own homes. NORCs facilitate and integrate existing community health and social services while organizing additional supports to enable older adults to remain in their community. Locations available throughout NYC. Works under general direction.

What You Will Do
  • Collaborate with site staff to ensure residents engage in self-care management health strategies.
  • Provide health-focused outreach to seniors who may benefit from supportive services.
  • Promote and encourage healthcare activities such as health screenings, assessments, linkages, and presentations. Make telephone calls and home visits to assess resident needs and support community living.
  • Make referrals to appropriate health services based on resident needs.
  • Act as a liaison between healthcare systems during transitions, providing health care advocacy.
  • Assist residents needing direct patient care to connect with their physicians or other health services.
  • Document all nursing interventions as required by the site.
  • Participate in special projects and perform other duties as assigned.
Qualifications

Licenses and Certifications: Current registration as a Registered Professional Nurse in New York State and CPR/BLS certification required.

Education: Associate's Degree in nursing required; Bachelor's Degree preferred.

Work Experience: Minimum of two years as an RN in LHCSA or CHHA. Prior experience with aging populations and clinical assessments required.

Pay Range

USD $40.95 - USD $51.19 / Hr.
