Job summary

Integral Care Home Care is seeking a Home care Field RN Supervisor. This per-diem role requires an active New Jersey RN license, a valid driver’s license, and strong organization to manage care plans for home-based patients.

Candidates should have 1–2 years of clinical nursing experience and excellent communication skills. The Field RN Supervisor will assess patients at start of care, initiate and modify plans of care, and supervise home health aides, ensuring quality home care delivery in

Responsibilities

Responsibilities: Field Nurse Superviser

• Assess patient at start of care to develop a plan of care
• Initiate Plan-of-Care and make necessary changes as patient’s needs evolve
• Evaluate patients in their homes
• Instruct and advise patients and/or families as well as the home health aides in patients' care
• Reassess patients every two months to determine any condition change as well as supervise the home health aide to ensure proper care

Benefits

• Hourly rate of $50-$100
• $100/hour for Initial assessment & reassessments
• +$25 for after-hours
• Office time $50/hour
• Visits to HHA $75.00/hour
• Weekend Visits - $75/hour
• On Call - $50 a day - in case there are calls.
• Mileage Reimbursed
