Responsibilities

Responsible for providing professional nursing care to patients in their home through assessment, development, implementation and evaluation of home nursing care needs on an individual and continuous basis of assigned patients in accordance with organization policies and procedures and external regulations.

• Assure compliance with physician orders and timely documentation, including progress toward discharge goals.
• Ensure assessment, planning, intervention and evaluation follow policy and standard of care, and that documentation reflects these activities.
• Apply clinical judgment and critical thinking in alignment with professional standards of care and codes of ethics.
• Identify appropriate payors and maintain compliance with authorizations, meeting payor guidelines and documentation requirements.
• Complete nursing assessment, medication reconciliation, and collaborate with physicians on Care Plans, updating within two weeks of status changes.
• Implement teaching strategies aligned with goals and document them appropriately.
• Report patient care issues to management or physicians promptly, documenting communication and solutions.
• Act as liaison among physicians, managers, and care team members to coordinate services effectively.
• Communicate Plan of Care during patient care transfers on a daily basis or as needed.
• Complete routine assignments accurately and independently, meeting productivity requirements.
• Supervise para‑professional home health aide staff, conduct visits within regulatory time frames, and participate in staff evaluations.
• Maintain awareness of updated standards and incorporate them into practice.
• Perform additional duties or projects as assigned, demonstrating flexibility.
• Adhere to HMH organizational competencies and standards of behavior.
• Lift, push, pull up to 50 lbs. and stand up to 8 hours per day.

Starting Minimum Rate

Minimum rate of $42.90 per hour.
