Job summary

Integral Care Home Care in New Jersey is seeking a Home Care Field RN Supervisor – Per Diem to support Monmouth County and surrounding areas. This role requires home care experience and an active NJ RN license, plus reliable transportation.

Responsibilities include assessing patients, updating care plans, and supervising home health aides. You will train caregivers at our Freehold office on a weekly basis, with the flexibility of per diem scheduling.

Qualifications

• Valid, current New Jersey Registered Nurse license.
• Current driver's license.
• Minimum of 1–2 years of clinical nursing experience.
• Ability to work independently and communicate well.

• Assess patients at the start of care and develop individualized plans of care.
• Update plans of care as patients’ needs change.
• Evaluate patients in their homes.
• Instruct and guide patients, families, and home health aides regarding patient care.
• Reassess patients every two months to identify changes in condition and supervise home health aides to ensure appropriate care.
• Attend weekly training at the Freehold office; provide caregiver training.

Service Area: Monmouth County and select areas of the surrounding counties.

Integral Care Home Care is expanding into Central Jersey. We are just getting started in this area and anticipate a growing number of new cases, with opportunities for additional work as we expand.

Home care experience is required.

Qualifications:

• Valid, current New Jersey Registered Nurse license
• Current driver’s license
• Organized and patient-focused
• Able to work independently
• Strong oral and written communication skills
• Minimum of 1–2 years of clinical nursing experience

Responsibilities:

• Assess patients at the start of care and develop individualized plans of care.
• Update plans of care as patients’ needs change.
• Evaluate patients in their homes.
• Instruct and guide patients, families, and home health aides regarding patient care.
• Reassess patients every two months to identify changes in condition and supervise home health aides to ensure appropriate care.
• Once you begin working with us, come into our Freehold office to provide caregiver training, initially one day per week. Availability for weekly office training is required.

Compensation and Benefits:

• Hourly rate: $50–$100
• Initial assessments and reassessments: $100/hour
• After-hours: additional $25
• Office time, including caregiver training: $50/hour
• HHA supervisory visits: $75/hour
• Weekend visits: $75/hour
• On-call: $50 per day when calls occur
• Mileage reimbursement
