Job summary

Allaire Health Services, LLC in New Jersey is seeking an experienced registered nurse for in-home patient care. You will assess patients at start of care, develop and adjust plans, and monitor progress in home settings.

Responsibilities include instructing families and home health aides, reassessing every two months, and ensuring high-quality, independent care delivery. A NJ RN license and current driver's license are required, with 1–2 years of clinical nursing experience preferred.

Qualifications

• Valid & current New Jersey Registered Nurse License and Registration.
• Current Driver's License.
• Organized and patient oriented.
• Able to work independently.
• Good oral and written communication skills.
• Minimum of 1-2 years clinical nursing experience.

Responsibilities

• Assess patient at start of care to develop a plan of care.
• Initiate Plan-of-Care and make necessary changes as patient's needs evolve.
• Evaluate patients in their homes.
• Instruct and advise patients and/or families as well as the home health aides in patients' care.
• Reassess patients every two months to determine any condition change as well as supervise the home health aide to ensure proper care.

Benefits

• Hourly rate of $50-$100
• $100/hour for Initial assessment & reassessments
• +$25 for after-hours
• Office time $50/hour
• Visits to HHA $75.00/hour
• Weekend Visits - $75/hour
• On Call - $50 a day - in case there are calls.
• Mileage Reimbursed
