Job summary

Integral Care Home Care in Central Jersey is seeking a Home Care Field RN Supervisor – Per Diem to support Monmouth County and surrounding areas. You will assess patients, develop and update care plans, and supervise home health aides in patients’ homes.

The role requires NJ RN licensure, valid driver’s license, 1–2 years of clinical nursing experience, and the ability to work independently with strong communication skills. Per diem schedule with office training in Freehold.

Qualifications

• Valid, current New Jersey RN license.
• Current driver’s license.
• 1–2 years of clinical nursing experience.
• Organized, patient-focused, able to work independently.
• Strong oral and written communication skills.

Responsibilities

• Assess patients at the start of care and develop individualized plans of care.
• Update plans of care as patients’ needs change.
• Evaluate patients in their homes.
• Instruct families and home health aides on patient care.
• Reassess patients every two months and supervise aides.
