Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations for health plan members. You’ll visit patients in their homes, review medications and medical history, and perform basic clinical assessments using an iPad.

The role is part-time with an hourly pay range of $37.66–$81.11, offering malpractice coverage, supplies, and incentive opportunities per visit. Travel locally within licensed states is expected as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required for the role.
• Multi-state licenses are preferred and advantageous.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Travel locally within licensed states as needed; conduct virtual visits.
• Use an iPad and clinical workflow; no prescribing or lab orders.
