Job summary

Signify Health, part of CVS Health, is seeking Per Diem Clinicians (NPs or PAs) to perform in-home health evaluations. The role involves visiting patients at home, reviewing medical history, and performing physical assessments with an iPad-based workflow.

You may conduct virtual visits and diagnostic services as needed. The position is part-time with a base hourly pay range and benefits for eligible per diem staff.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations in patients' homes.
• Use iPad and clinical workflow during evaluations.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Provide diagnostic and preventive services as indicated (e.g., spirometry, etc.).
