The LPN will provide premier services to medically involved individuals and participate in daily routines, activities, and medication administration.

Qualifications

• MA state valid LPN or RN license.
• Criminal background check may be required and a valid driver’s license.
• TB screening and physical upon hire with periodic updates.

Responsibilities

• Distribute medication and provide ADL care while maintaining dignity.
• Deliver a structured daily routine including activities and visits.
• Implement Positive Behavior Support and Individual programs as identified.
