Qualifications

• MA state LPN or RN license required.
• Background check and valid driver’s license required.
• TB screening and Physical upon hire; TB update every two years after hire.

Responsibilities

• Provide a structured daily routine including house meetings, individual outings, meal preparation, ADL care, family visitations, and medication distribution while upholding human rights.
• Implement Positive Behavior Support and ISP-based individual programs.
• De-escalation and crisis management as needed to maintain safety.
