Horizon Health is seeking a per diem LPN for our opioid clinic in High Point, NC. The LPN coordinates and evaluates patient care through the interdisciplinary team, following the nursing process and OTP standards of care.

The role involves dispensing methadone, accurate documentation, patient assessments, surveillance, and education, with participation in team meetings. 2+ years in addiction care and NC LPN license required; Bachelor's degree preferred.

Qualifications

• Current NC LPN license.
• Minimum 2 years experience in an addiction care setting.
• Bachelor's degree preferred.

Responsabilités

• Dispenses Methadone and other medications per physician orders and clinic policy.
• Record all patient information accurately in the patient record.
• Complete brief assessment of mental and physical well‑being and monitor for drug use signs.
• Participate in UDS surveillance per policy.
• Coordinate medication reviews with agency physicians.
• Complete all required controlled substance inventories.
• Conduct patient education programs including health and wellness education.
• Participate in treatment team meetings and provide feedback.
