Job summary

Horizon Health is seeking an LPN for Foundations Health in High Point, NC to support opioid treatment programs on a per diem basis. The LPN delegates, coordinates, and evaluates patient care with the interdisciplinary team in accordance with OTP standards.

The role requires dispensing methadone per physician orders, accurate charting, and active participation in patient education, treatment planning, and team meetings within approved clinic policies.

Qualifications

• Current NC LPN license.
• Bachelor's degree preferred.
• Minimum 2 years experience in an addiction care setting preferred.

Responsibilities

• Dispenses Methadone and other medication, per physician orders and per clinic policy
• Accurately record all patient information in the patient record
• Completes brief assessment of patient's mental and physical well-being, monitoring for signs/ symptoms associates with licit and illicit drug use
• Participates in UDS surveillance, per policy
• Coordinates medication reviews and staffing with agency physicians
• Completes all required controlled substance inventories to ensure accurate medication counts
• Conducts patient education programs according to needs identified, including health and wellness education
• Participates in treatment team meetings, providing feedback to the interdisciplinary team
