Job summary

Horizon Health is seeking an LPN for Foundations Health in High Point, NC. The OTP LPN delegates, coordinates, and evaluates patient care through collaborative efforts with the interdisciplinary team, following nursing process and OTP standards of care and practices.

Responsibilities include dispensing methadone per physician orders, recording patient information, brief assessments, UDS surveillance, coordinating reviews, inventories, patient education, and participating in treatment team

Qualifications

• Current NC LPN license.
• Minimum 2 years experience in an addiction care setting.
• Bachelor's degree preferred.

Responsibilities

• Dispenses Methadone and other medication, per physician orders and per clinic policy
• Accurately record all patient information in the patient record
• Completes brief assessment of patient's mental and physical well-being, monitoring for signs/ symptoms associated with licit and illicit drug use
• Participates in UDS surveillance, per policy
• Coordinates medication reviews and staffing with agency physicians
• Completes all required controlled substance inventories to ensure accurate medication counts
• Conducts patient education programs according to needs identified, including health and wellness education
• Participates in treatment team meetings, providing feedback to the interdisciplinary team
• Schedule: Monday-Friday 5am-2pm
