Qualifications

• High School Diploma or equivalent; LPN/LVN licensure required.
• Minimum one year of nursing or caregiver experience; preferably in assisted living, home health, or memory care.
• Minimum six months experience in a supervisory role.
• Must be able to read physician orders and match to MARs.
• Excellent verbal and written communication skills; able to communicate in person and by phone.

Responsibilities

• Follows the services outlined in each resident's service plan and documents changes per policies.
• Communicates resident needs to the Director of Health and Wellness as needed to ensure quality services.
• Participates in providing hands-on care and supports the resident's care plan; reviews plans for regulatory compliance.
• Provides clinical support and training of care staff for continuing education requirements.
• Assists in transcription of physicians' orders and review of new orders.
• Maintains required certifications and participates in in-service trainings and accreditations.
