Job summary

Emerald Healthcare in Claremore, OK is seeking an LPN to provide compassionate nursing care in Memory Care. You will assess patient needs, deliver direct nursing care, and collaborate with the healthcare team to ensure optimal resident outcomes.

You'll work in a supportive skilled nursing facility, with PPE provided. Applicants should have an LPN/LVN license and CPR/BLS, plus ability to work with a diverse team and families.

Qualifications

• Licensure: Valid LPN/LVN license in New York.
• Education: Graduation from an accredited LPN/LVN program.
• Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.
• Certifications: Current CPR/BLS certification required; other certifications may be required by state regulations.
• Communication: Excellent verbal and written communication skills and teamwork.
• Physical: Ability to lift/move patients and stand for extended periods.

Responsibilities

• Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
• Monitor residents’ vital signs, document progress, and report significant changes to the Charge Nurse or Director of Nursing.
• Assist in developing and implementing individualized care plans tailored to each resident’s needs.
• Maintain clear and accurate documentation of patient care and communicate with residents, families, and the healthcare team.
• Adhere to facility policies, infection control and safety protocols.
• Provide emotional support and education to residents and their families, assisting with understanding care plans.
• Collaborate with RNs, CNAs, physicians, and therapists to deliver coordinated and comprehensive care.
