St. John’s is a leading eldercare provider in Rochester, NY, inviting compassionate LPNs to join our team for per-diem all shifts. The role focuses on nursing care for Elders in a family-like environment with close nurse collaboration and DOH-compliant practice.

You will perform licensed nursing interventions, administer medications, assist with transfers, and monitor Elder health status under RN oversight. We value teamwork, respect, and compassionate care for residents.

Qualifications

• Current LPN license in good standing with NYS DOH.
• Good verbal and written communication skills.

Responsabilités

• Nurture and support Elders with high-quality nursing care.
• Administer medications and document treatments as ordered by the physician.
• Notify RN, Case Manager or on-call about health status changes.
• Provide direct care including personal care and transfers as needed.
• Monitor medication regimes and report adverse reactions.
• Collaborate with RN Case Manager on the medication management plan.
