St. John's Lutheran Ministries, Inc. DBA St. John's United seeks a licensed practical nurse to provide high-quality nursing care at WyndStone. You will collaborate with medical staff and families to ensure safe, compassionate elder care.

You will assess residents, document care plans, respond to emergencies, and participate in care coordination with the administration and health director.

Competenze

• Must be licensed as LPN in Montana and in good standing.
• Prior health care experience is preferred.
• Completed a state-certified LPN program or equivalent.
• Ability to interact professionally with Elders, families and staff.

Mansioni

• Assess Elders and oversee nursing treatments with providers.
• Maintain professional standards and care continuity for Elders.
• Identify changes in condition and communicate with providers.
• Respond to and lead emergency situations.
• Keep accurate nursing notes, vitals and incident reports.
• Develop Care Plans and communicate with families.
• Assist Admit/Discharge and coordinate with case management.
• Attend team meetings and ensure cross-shift collaboration.
• Work with Administration to improve elder care quality.
• Other duties as assigned.
