Job summary

St. John’s Health Care Corporation is seeking a Licensed Practical Nurse (LPN) for part-time and full-time roles across Day, Evening, and Night shifts.

Pay ranges from $31.00 to $35.00 per hour and increases with licensure; evening and weekend shifts offer additional compensation. The LPN will provide nursing care and support for elders in a home setting, ensuring safety, cleanliness, meals, and daily living needs are met in accordance with each care plan.

Qualifications

• Must be a Graduate of an approved school of practical nursing with current New York State Practical Nursing License.
• CPR certification required.
• Valid NYS Driver’s License preferred (required for Green Houses).
• Long-term care experience preferred.
• Good organizational, leadership and communication skills.
• Desire and ability to interact with elders and care for their needs.
• Willingness to embrace and exemplify St. John’s Brand Characteristics.

Responsibilities

• Respond to elder needs and preference; elder contentment is a priority.
• Promote and support autonomy of elders in decision making.
• Contribute to the nursing process and implement elder’s plan of care.
• Safely perform direct nursing services such as administering medications and treatments, providing personal care and activities of daily living.
• Actively participate as a member of the care team, communicating with needed team members and documenting as needed.
• Meet personal care needs of elders.
• Consult and validate data with the RN, record and report pertinent information, provide explanations to families and other staff.
• Care for personal belongings such as clothing, dentures, eyeglasses, hearing aids, prostheses.
• Recognize and report safety hazards and elders’ health problems and responses to treatment.
• Assist with therapeutic and diagnostic procedures.
• Plan and support activities for people living in the home.
• Maintain privacy and confidentiality of elder care.
• Prepare and serve meals and snacks based on menu and preferences.
• Innovate and work autonomously to improve elders’ life.
• Actively promote the Small Home philosophy through behaviors and communications.
