Masonicare is seeking a Per Diem Licensed Practical Nurse to provide nursing care at Wesley Heights in Shelton, CT. Under supervision, you will support resident services plans, collaborate with the ALSA Director/RN Coordinator, and assist with medication management and education.

You will work with residents, families and the health care team to ensure quality care, accurate documentation, and ongoing professional development through training and workshops.

Qualifications

• Graduate of accredited School of Nursing.
• Current LPN license; CPR certification.
• 1-2 years clinical experience preferred.

Responsabilités

• Contributes to resident care and service plans by collecting data and participating in care planning meetings.
• Communicates and collaborates with residents, families, colleagues and health care team to meet resident needs.
• Provides education, supervision and counseling to residents and families regarding AL procedures and other care needs.
• Documents accurate and timely information in the client service record.
• Oversees medication management with supervision and in collaboration with physicians and pharmacists.
