Job summary

The Elms Rehabilitation and Skilled Nursing in Milford, NH is seeking per-diem LPNs and RNs to provide high-quality care to residents on an as-needed basis. Positions offer flexible shifts and the opportunity to work with a supportive care team.

Responsibilities include medication administration, monitoring patient conditions, and collaboration with the healthcare team to adjust care plans. NH licensure or a valid multistate compact license with NH authority is required.

Qualifications

• Valid LPN or RN license in NH or under a multistate compact with authority to practice in NH.
• Strong clinical and compassionate patient care.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Experience in long-term care preferred but not required.

Compensation: LPN $36.00 - 44.00 | RN $44.00 - 49.00 based on experience

Shifts: Per-diem team members have the flexibility to pick up shifts as needed

Requirements

• Previous experience in long-term care or a similar setting is preferred but not required.
