Comprehensive-Community-Action (CCAP) seeks an independently licensed clinician for the Family Care Community Partnership program in Rhode Island. The role involves initial diagnostic evaluations, constructing clinical formulations, and mapping ongoing care plans with input from FSCC staff.

The per diem, fee-for-service position requires flexibility for evenings/weekends and home-based services to serve the target population. A strong license and bilingual skills are a plus.

Qualifications

• Master's degree in social work or related field.
• Unrestricted license to practice in Rhode Island (LICSW, LMFT or LMHC).
• Excellent written and oral communication skills.
• Provide immunizations/vaccinations as required.
• Complete all background checks.
• Must hold a valid driver’s license and have proof of insurance.
• Bilingual Spanish a plus.

Responsabilités

• Approves required documentation in DCYF-RIFIS system.
• Day 30: Review/sign CANS; complete family visit and D/Sed/Determin. (DI)
• Day 60: Review/sign SNCD.
• Day 90: Review Family Plan and sign; continue every 90 days.
• Complete all written work, evaluations, progress notes timely.
• Oversee FCCP records per CCAP policies and DCYF standards.
• Assist supervisor with development of treatment/services per DCYF billing.
• Ensure all documents comply with Medicaid/state standards.
• Other duties as assigned.
