Bristol Hospice is seeking a compassionate LPN/LVN to join our Gunnison team. Provide direct patient care and end-of-life support in a hospice setting, with emphasis on comfort, education, and family involvement.

Position is PRN hourly in Gunnison, UT; flexible shifts required, with responsibilities including care plan implementation, documentation, and collaboration with the interdisciplinary team.

Qualifications

  • Must hold a current LPN/LVN license in the state of Utah.
  • Minimum three years nursing experience; hospice or medical/surgical preferred.
  • Ability to apply hospice philosophy and address end-of-life needs.
  • Flexible schedule and willingness to adapt to shift changes.
  • Comfort with documenting care plans and using information systems.

Responsibilities

  • Provide direct patient care per State Nurse Practice Act and end-of-life duties.
  • Implement care plans based on assessments.
  • Document patient services accurately and timely.
  • Educate patients and families about diagnoses and self-care.
  • Coordinate hospice services with interdisciplinary team members.
  • Utilize equipment properly and maintain aseptic technique.
  • Manage pain and symptom relief and monitor responses.
  • Contribute to ongoing quality improvement efforts.

Qualifications

An Average Day: (Includes, but not limited to)

  • Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
  • Implement current nursing practice following a comprehensive assessment and the plan of care
  • Provide accurate and timely documentation of patient services to reflect the plan of care
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
  • Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
  • Use and prepare equipment and supplies effectively, adhering to aseptic technique
  • Provide appropriate pain/symptom management
  • Evaluate patients response to treatments/medications
  • Participate in personal, professional growth and development
  • Participate in organization's quality assessment performance improvement teams and activities
  • Perform other duties as assigned by the registered nurse
  • All other duties as assigned
Requirements
  • Must currently be licensed as an LPN/LVN in the State
  • Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
  • Must understand the philosophy of hospice concept and understand needs of the terminally ill
  • Must be flexible in work hours
  • Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
