Bristol Hospice Fresno is seeking a compassionate Licensed Vocational Nurse (LVN) for hospice care in Fresno, CA. This PRN position offers an hourly pay range of $29.00–$31.00 with a graveyard shift and opportunities to deliver high-quality, patient-centered care.

Qualifications

• Must be licensed as an LPN/LVN in the state.
• Minimum 3 years nursing experience; hospice or medical/surgical preferred.
• Must understand hospice concepts and needs of terminally ill patients.
• Must be flexible in work hours.
• Must maintain knowledge of information systems and software used.

Responsibilities

• Provide direct patient care as per State Nurse Practice Act, including end-of-life duties.
• Implement current nursing practice based on assessments and care plans.
• Document patient services accurately and timely to reflect the plan of care.
• Educate patients and families about diagnoses and self-care.
• Coordinate hospice services and report needs to the interdisciplinary team.
• Assist with equipment and supplies, ensuring proper aseptic technique.

Qualifications

An Average Day: (Includes, but not limited to)

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements

• Must currently be licensed as an LPN/LVN in the State
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
