Job summary

Bristol Hospice Fresno, CA is seeking a compassionate Licensed Vocational Nurse (LVN) to join our team on a PRN basis in Fresno.

This role provides direct patient care in end-of-life settings, follows the plan of care, documents services, and collaborates with the interdisciplinary team. The position requires state LVN license, at least three years of nursing experience, and flexibility in hours; graveyard shift is typical.

Qualifications

• Must currently be licensed as an LPN/LVN in the State.
• Must have three (3) years nursing experience; hospice or medical/surgical preferred.
• Must understand the philosophy of hospice and care for the terminally ill.
• Must be flexible in work hours.
• Must maintain knowledge of information systems and applicable software.

Responsibilities

• Provide direct patient care per State Nurse Practice Act, including end-of-life duties.
• Implement current nursing practice following the plan of care.
• Document patient services accurately to reflect the plan of care.
• Educate patients and family/caregivers about diagnosis and self-care.
• Coordinate hospice services with the interdisciplinary team.
• Prepare equipment and supplies, adhering to aseptic technique.
• Provide pain/symptom management and monitor responses.
• Participate in quality improvement activities and professional growth.
• Perform other duties as assigned by the registered nurse.

Qualifications

An Average Day:

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements

• Must currently be licensed as an LPN/LVN in the State
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
