Responsibilities

• Delivers patient care by applying the nursing process and professional practice standards.
• Assesses and evaluates patient care needs with critical thinking in care management.
• Integrates assessment and intervention skills in nursing delivery.
• Collaborates with the interdisciplinary healthcare team on the plan of care.
• Prioritizes patient care including teaching, rounding, and care planning after discharge.
• Educates patients and caregivers about their plan of care and transition of care.
• Communicates effectively with patients, families, and the care team.
• Manages assignments within Modified Primary Nursing and allocates resources.
